Requirements to Register New Students
WELCOME TO BRIDGES CHARTER SCHOOL ENROLLMENT PROCESS
Registrations for TK through 8th grade for the school year 2025-2026 begin on Monday, March 24, 2025.
Please bring the following documentation to the school office to complete your registration:
1. Verification of Student Date of Birth:
Please submit one of the following:
✓ A copy of the student's birth certificate
✓ A statement by the local registrar or county recorder certifying the date of birth for the student
✓ A record of birth certificate date from a Health Office/Vital Statistics
✓ A duly attested baptism certificate for the student
✓ An active (not expired) passport for the student
✓ A Court Order showing the student's name, date of birth, and guardianship status
*If the document does not contain the parent/guardian's name, additional guardianship verification is required.
*If none of the above documents are obtainable, an affidavit of birth must be signed by the parent/guardian.
2. Verification of Immunizations -
Required immunizations (shots or vaccinations). All students must have documentation on file showing they are up to date on immunizations, or they may not be able to attend school. Bring the immunization record at the time of enrollment. The following immunizations are required for school entry:
K-12 students
✓ Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses (4 doses OK if one was given on or after 4th birthday. 3 doses OK if one was given on or after 7th birthday)
✓ Polio (OPV or IPV) — 4 doses (3 doses OK if one was given on or after 4th birthday)
✓ Hepatitis B — 3 doses (not required for 7th grade entry)
✓ Measles, Mumps, and Rubella (MMR) — 2 doses (first dose given on or after 1st birthday)
✓ Varicella (chickenpox) — 2 doses
Students entering 7-12 grade
✓ Tetanus, Diphtheria, Pertussis (Tdap) —1 dose
✓ Varicella (chickenpox) — 2 doses
- Please note that the TK-12 immunization requirements apply to 7th graders who previously had a valid personal beliefs exemption (PBE) filed in California before 2016. Pre-existing PBEs expire upon entering 7th grade and, therefore, the student is required to meet the TK-12 requirements.
Exemptions:
- Personal belief or religious exemptions are no longer accepted. Valid medical exemptions are acceptable.
- Medical exemptions must be submitted through CAIR-ME for any vaccine that is contraindicated

For religious or personal belief exemptions:
As of January 1, 2016, the California Health and Safety Code Sec. 120335 (b) states that a private or public elementary or secondary school shall not admit a student unless the student has been fully immunized. Since the change in the law, exemptions for religious or other personal beliefs are no longer an option for the required vaccines for entry into school in California.
3. Proof of Residency (TWO of the following are required) - Document must be dated within 30 days :
✓ Gas, Water, Electric, rent receipt or a lease agreement. Bill must have parent’s name and date on the document. If unable to provide proof of residence, ask for an Affidavit of residence and return with a proof of address of person who signed the affidavit.
All children who reside in the United States are guaranteed access to a free public education. No family will be denied access to school because of their immigration status.
4. Oral Health Assessment:
✓ Only for Transitional Kindergarten and Kindergarten
5. Legal Custody papers – if there are any legal court orders or foster placement papers.
6. Special Education or 504 Plan:
✓ If your child has an Individualized Education Plan or a 504 Plan
7. Physical Examination (CHDP): No longer required
Physical examination for any TK, kindergartener or new first grader. As of July 1, 2024,the kindergarten/first grade physical exam requirement is no longer required for school entry, however, it is still strongly recommended. If available, please submit documentation of your child's physical exam on the School Health Form or to the front office or upload to student documents.
8. Medication Form - If your child will be taking any kind of medication at school, you will need to complete the school Medication Form, have it signed by a doctor and turn it in to the front office as soon as possible or before the first day of school or before any medication can be administered at school. This includes over-the-counter as well.

All registration/enrollment packets and paperwork must be completed by May 7th, 2025
or within 2 weeks of acceptance of an offer of a spot in our program if after the original lottery in March 2025 for the school year 2025-2026. If we have not received all of your completed enrollment paperwork by the deadline, your spot may be offered to the next person on the waitlist, and your name may be moved to the bottom of the list.
